As a courtesy, we will file insurance claims to most insurance companies. To assist with this service, we ask that you carefully and completely fill out our Patient Information Form and Medical Orthopaedic History Form.

We accept assignment of benefits, meaning direct payment from your insurance company. You will be required to fulfill co-pay obligations and your estimated co-insurance, that which is owed at the time services are rendered and that which the insurance company will not cover. If your insurance company has not completed or cooperated in the payment of your account within 90 days, the balance will become your responsibility. It is therefore important for you to understand what your insurance company covers and to follow up on their payment of your claims.

Should any necessary adjustments or changes be made regarding your insurance information, please let us know immediately.

If you cannot make a scheduled appointment, please give us as at least 48 hours to arrange that time for another patient.

Remember, narcotics are not called in after hours. Please make arrangements for prescription refills during normal business hours, and make note that we close at 1:00 pm on Friday afternoons.
